Client Certification

•United Christian Outreach ONLY SERVES PEOPLE WHO LIVE WITHIN THE CITY LIMITS OF LAFAYETTE

1. Proof of Identity: Current driver's license or pictured I.D. for each adult in the household

2. Proof of Address a recent utility or phone bill with a current address

3. Current Income of all adults in the household

4. If applicable: Proof of SSI benefits or SNAP benefits

5. Names, dates of birth, ages for every member of the household
